Provide specialized Physical Medicine and Rehabilitation services to patients within skilled nursing facilities. Collaborate with interdisciplinary teams, including physical and occupational therapists, to optimize patient functional outcomes and independence.
Candidates must hold a valid Nurse Practitioner, APRN, or Physician Assistant license in Illinois. Experience in the post-acute care space is preferred, along with a passion for serving the geriatric population.
