About the Role

Key Responsibilities

• Responsible for teaming with the Project Director in implementing a behavioral health and health initiative.

• leads project integrated care teams composed of clinical leaders, psychiatry, case managers, medical assistant, as needed.

• Conducts and reviews health care screenings and assessments. Ensures client linkage to primary and specialty care as needed, working in collaboration with project staff.

• Other duties as assigned.

Requirements

• Bachelor’s Degree in Nursing/RN license with 1

• 2 years of related experience in behavioral health or integrated care, or Licensed Practical Nurse (LPN) with 2

• 4 years of related experience including staff supervision.

• Strong belief and commitment to integrated care models; Knowledge, experience and sensitivity to working with clients with behavioral health, addiction and co
