About the Role
Join Catholic Charities as a Behavioral Health Program Nurse Care Manager – CCBHC in United States.
Key Responsibilities
- Responsible for teaming with the Project Director in implementing a behavioral health and health initiative.
- leads project integrated care teams composed of clinical leaders, psychiatry, case managers, medical assistant, as needed.
- Conducts and reviews health care screenings and assessments. Ensures client linkage to primary and specialty care as needed, working in collaboration with project staff.
- Other duties as assigned.
Requirements
- Bachelor’s Degree in Nursing/RN license with 1
- 2 years of related experience in behavioral health or integrated care, or Licensed Practical Nurse (LPN) with 2
- 4 years of related experience including staff supervision.
