About the Role

Join Catholic Charities as a Behavioral Health Program Nurse Care Manager – CCBHC in United States.

Key Responsibilities

Responsible for teaming with the Project Director in implementing a behavioral health and health initiative.

leads project integrated care teams composed of clinical leaders, psychiatry, case managers, medical assistant, as needed.

Conducts and reviews health care screenings and assessments. Ensures client linkage to primary and specialty care as needed, working in collaboration with project staff.

Other duties as assigned.

Requirements

Bachelor’s Degree in Nursing/RN license with 1

2 years of related experience in behavioral health or integrated care, or Licensed Practical Nurse (LPN) with 2

4 years of related experience including staff supervision.

Strong belief and commitment to integrated care models; Knowledge, experience and sensitivity to working with clients with behavioral health, addiction and co
