About the Role
Are you a PMHNP who wants to practice psychiatry the way it should work — embedded alongside primary care, behavioral health clinicians, and care managers, not siloed in a separate system?
Thundermist Health Center is looking for a Psychiatric Mental Health Nurse Practitioner to join our integrated behavioral health team.
You’ll conduct psychiatric evaluations, manage complex medication regimens, provide MAT for substance use disorders, and collaborate daily with LICSWs, PCPs, pediatricians, and community partners.
Your patients are diverse, largely medically underserved, and many have co-occurring medical and behavioral health conditions. No after-hours call, no hospital coverage — just meaningful, team-based psychiatric care in a community that needs you.
What You’ll Do
Psychiatric Clinical Care
Conduct comprehensive psychiatric evaluations including mental status exams, risk assessments, substance use screening, and diagnostic formulation using DSM-5-TR and ICD-10
Develop, implement, and update individualized psychiatric treatment plans in collaboration with patients and the care team
Prescribe, titrate, and monitor psychiatric medications including controlled substances; adjust regimens based on clinical response and tolerability
Provide Medication-Assisted Treatment (MAT) for substance use disorders, including buprenorphine induction and maintenance
Assess clinical emergencies including suicide risk and risk of harm to others; determine appropriate level of care and coordinate transitions
Provide brief supportive counseling, psychoeducation, and motivational interviewing within the scope of medication management visits
Integrated Care & Collaboration
Collaborate within Thundermist’s integrated behavioral health model alongside LICSWs, PCPs, pediatricians, care managers, and community partners
Participate in warm hand-offs from primary care and behavioral health for new psychiatric evaluations and medication consultations
Provide curbside consultation to primary care providers on psychiatric presentations, medication questions, and risk assessment
- Participate in team-based care conferences, complex case reviews, and care management rounds
- Coordinate care with external psychiatric providers, community mental health centers, inpatient facilities, and SUD treatment programs
Documentation & Compliance
Complete clinical documentation in the EHR (eClinicalWorks) accurately and within same-day charting requirements
Ensure compliance with HIPAA, 42 CFR Part 2 (SUD confidentiality), Rhode Island APRN scope of practice, and DEA prescribing requirements
Support HRSA UDS reporting for behavioral health measures (depression screening and follow-up, SBIRT, tobacco cessation counseling)
Participate in chart audits, peer review, and quality improvement activities
What We’re Looking For
You’re a psychiatric NP who’s as comfortable managing a complex polypharmacy regimen as you are sitting with a patient in crisis. You value team-based care, believe in meeting patients where they are, and want your work to directly advance health equity.
- MSN or DNP from an accredited PMHNP program
- Current PMHNP-BC certification through ANCC
- Full, unrestricted Rhode Island APRN licensure with PMHNP specialty
- Current DEA registration and RI Controlled Substance Registration with full Schedule II–V prescriptive authority
- MAT/buprenorphine prescribing authority (or willingness to obtain within established timeframes)
Current BLS Certification
- Eligibility to enroll with Medicare, Medicaid, and commercial payers (current CAQH profile required)
- Demonstrated proficiency in psychiatric evaluation, psychopharmacology, and clinical emergency assessment
Preferred
- Prior experience in an FQHC, community mental health center, or safety-net behavioral health setting
- Experience with integrated behavioral health / primary care behavioral health (PCBH) models
- Experience with MAT including buprenorphine induction and maintenance
- Experience with perinatal mental health, pediatric/adolescent psychiatry, geriatric psychiatry, or trauma-informed care
- Bilingual (Spanish/English or Portuguese/English) strongly preferred
- Eligibility for NHSC or Rhode Island state loan repayment programs

Schedule & Location
Full-time (40 hours/week), benefit-eligible, based at an assigned Thundermist location. Monday through Friday during clinic hours, with some evening sessions to support patient access. Visits are primarily in-person with telehealth available as clinically appropriate. Occasional float coverage across other Thundermist sites may be required. No after-hours call or hospital inpatient coverage.
