Job Description

The Social Worker LMSW conducts psychosocial evaluations, care management services and discharge planning. Develops and implements patient treatment plans and participates in treatment team conferences. Relies on education, experience and judgment to accomplish job. Works under supervision required by regulation and licensing board. Creativity and some latitude is expected to complete responsibilities.

Responsibilities

We are seeking a compassionate patient advocate, resourceful, and highly organized Community-Based Social Worker who will operate as a placement specialist to support individuals and families navigating the complex process of long-term care placement.

In this role, you will serve as a trusted resource and advocate for clients and their families, helping them understand care options, financial considerations, insurance coverage, eligibility requirements, and the steps involved in transitioning to an appropriate care setting.

You will work closely with families, healthcare providers, nursing facilities, community organizations, and other partners to coordinate services and help ensure a smooth and informed placement process.

You will also develop and maintain strong referral relationships with physician offices, clinics, rehabilitation centers, home health agencies, hospice providers, and other healthcare organizations.

Identifies opportunities to expand referral networks, maintains ongoing communication with referral sources and community partners, and distributes marketing materials and education regarding placement services through presentations and educational programs and represent the organization at community events, health fairs, networking events, and professional meetings.

Key Responsibilities

  • Meet with individuals and families to assess long-term care needs and discuss available nursing home placement options.
  • Conducts psychosocial assessments and gathers relevant medical, social, and financial information; educates clients and families regarding the placement process, levels of care, and community resources.
  • Assists families in identifying appropriate nursing facilities (available levels of care) based on clinical, financial, geographic, and personal preferences, and coordinates referrals and admissions to nursing homes and long-term care facilities.
  • Serves as a liaison between families, healthcare providers, nursing facilities, and community agencies, advocating for timely access to appropriate long-term care services and maintaining regular communication throughout the placement process.
  • Provide individualized case management, education, advocacy, and ongoing support throughout the placement process.
  • Identify barriers to placement and develop solutions in collaboration with clients, families, and referral partners.
  • Coordinate communication among clients, family members, healthcare providers, nursing facilities, and community resources.
  • Assist with gathering required documentation and completing appropriate applications, referrals, and placement-related paperwork.
  • Follow up with clients and families to monitor progress, address barriers, and ensure continuity throughout the placement process.
  • Maintain accurate, timely, and confidential case records in accordance with organizational and regulatory requirements.
  • Experience providing case management, social services, care coordination, or client advocacy.
  • Excellent communication, documentation and computer skills, interpersonal, organization, and problem-solving skills to work effectively with individuals, families, healthcare professionals, and community partners.
  • Ability to manage multiple cases, priorities, and deadlines independently.

Qualifications

Master's Degree

Crisis Intervention Training within 90 days of employment

Proficient in the English language, verbal and written communications skills, basic computer skills Current Louisiana State Board of Social Worker License (CSW or LMSW)
