Key Responsibilities
The physical therapist will conduct short-term in-home rehabilitation visits for orthopedic patients to facilitate their transition to outpatient care. Responsibilities include performing evaluations and follow-up visits while maintaining a caseload of approximately 6-7 patients per day.
Requirements
Candidates must be graduates of an accredited physical therapy program and hold an active or pending state physical therapist license. The role requires the ability to work independently in a home health setting.
