The RN Case Manager conducts comprehensive physical, social, and psychological assessments to create individualized care plans for patients in their homes. They manage care delivery, maintain meticulous patient records, and act as a primary resource for families.
Candidates must hold an active Registered Nurse (RN) license in Arizona and possess a valid driver's license with reliable transportation. Previous experience in Home Health or Hospice is preferred, along with a current CPR certification and an AZ Fingerprint Clearance Card.
