Job Description
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. In this role, you will provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans.
This position is carried out under the direction and supervision of the Executive/Regional Director and plays a critical role in enhancing patients' overall health, comfort, and quality of life.

The RN Case Manager provides skilled nursing care to patients in their homes according to physician orders and individualized care plans. They work under the direction of the Executive or Regional Director to improve patient health outcomes and quality of life.
Requirements
Candidates must be a licensed Registered Nurse (RN) with the ability to provide compassionate and skilled care. The role requires the ability to follow medical directives and manage patient care plans effectively in a home health setting.
