The Registered Nurse Case Manager (RN) plans, organizes, and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Responsibilities include but are not limited to:
Completes an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
- Completes initial evaluations, consents, and admissions for patients.
- Responsible for coordinating patient care for assigned caseload.
- Provides professional nursing care by utilizing all elements of nursing process.
- Assesses and evaluates patient’s status by: o Writing and initiating plan of careo Regularly re-evaluating patient and family/caregiver needso Participating in revising the plan of care as necessary
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
- Must complete weekly visit minimum as established by Office Administrator.
Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Work closely with patients on a 1-1 level but get strong support from a patient care team.
Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
Time management skills to manage visits, documentation, meetings (i. e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
Ability to learn an electronic medical records system.
Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area

The Registered Nurse Case Manager plans, organizes, and directs hospice care for patients within their homes and communities. Responsibilities include conducting comprehensive patient assessments, developing and revising care plans, and coordinating care with an interdisciplinary team.
