The Nurse Practitioner will assess patients' medical and mental health needs to develop and implement proactive care plans. They will also collaborate with an integrated care team to monitor patient responses and ensure compliance with clinical regulations.
Candidates must hold an FNP, AGNP, AHACNP, or PA license and possess a degree from an accredited program. A minimum of 3 years of clinical experience in geriatric populations is required.
