About the Role

Join Confidential Employer as a Social Worker – $23-$26/hr in United States.

Key Responsibilities

Upon admission the social worker will complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.

Facilitate Care Plan meeting with resident/family and interdisciplinary team admission quarterly, annually and significant changes.

Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long

term care facility, including their social, emotional, and psychological needs.

Requirements

Bachelors degree in Social Work, Human Services, or Psychology degree with experience in geriatrics

Must have one year supervised social work experience (paid/volunteer) in a health care setting working directly with individuals

Salary: $23 - $26/hr
