Job Description
Job Summary
Responsible for independently evaluating, planning, and implementing evidence-based physical therapy interventions to improve mobility, function, and quality of life for patients with a variety of diagnoses. Develops individualized treatment plans based on assessment findings and patient-centered goals.
Demonstrates advanced clinical reasoning in delivering care, managing caseloads efficiently, and collaborating with interdisciplinary teams. Provides education to patients, caregivers, and staff while maintaining compliance with professional, ethical, and organizational standards.
Essential Functions
- Performs comprehensive physical therapy evaluations and develops individualized treatment plans to address functional limitations and rehabilitation goals.
- Implements evidence-based therapeutic interventions including exercise prescription, manual therapy, modalities, and functional mobility training.
- Assesses patient progress, modifies treatment plans as needed, and communicates changes to the healthcare team.
- Educates patients and caregivers on home programs, safety precautions, and strategies to maximize independence and recovery.
- Collaborates with physicians, nurses, and other therapists to ensure coordinated, multidisciplinary care.
- Delegates appropriate treatment responsibilities to Physical Therapist Assistants and supervises students in accordance with state regulations.
- Maintains accurate, timely, and complete documentation of evaluations, interventions, and patient progress.
- Participates in departmental quality improvement initiatives, continuing education, and professional development activities.
- Ensures compliance with ethical standards, licensure requirements, and organizational policies.
- Performs other duties as assigned.

2-3 years of experience
An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part-time and PRN experience will be prorated based on hours worked per week. Volunteer work and internships for academic credit are not counted.
Certifications & Licensures
Maintains active licensure as a Physical Therapist. Achieves and maintains American Heart Association Basic Life Support (BLS)
