Join Sky Lakes Medical Center as a Physical Therapist - OP/IP - FT DAYS in United States. QUALIFICATIONS Required: Current Oregon licensure as a Physical Therapist or eligibility and evidence of application for licensure. Pelvic floor physical therapists will have completed at least level 1 training through a training institution widely recognized by the profession.
