Key Responsibilities

• Evaluate and treat patients of all ages.

• Develop individualized plans of care that include treatment goals, expected outcomes, and discharge planning.

• Modify the plan of care based on patient response, progress, and available clinical evidence.

• Perform patient transfers with assistance as needed from bed, chair, and floor when applicable. REQUIRED

Requirements

• Bachelor’s degree in Physical Therapy.

• Current Illinois Physical Therapist license.
