Promote timely and effective communication between patients, families, physicians, referral sources, caregivers, and members of the interdisciplinary team.
Ensure appropriate staffing and caseload management while maintaining high standards of care.
Participate in survey preparation and maintain ongoing survey readiness.
Develop and reinforce policies, procedures, and clinical standards that support consistent, high-quality care.
Foster a positive culture where employees feel supported, accountable, and empowered to provide excellent patient care.
Qualifications
Current, unrestricted Registered Nurse (RN) license in the applicable state.
Minimum of 3 years of nursing experience, with home health experience required.
Prior nursing leadership, supervisory, clinical management, or Director of Nursing experience preferred.
Strong understanding of home health clinical operations and regulatory requirements.
Knowledge of Medicare Conditions of Participation, OASIS, care planning, documentation standards, and quality improvement processes preferred.
Demonstrated ability to lead, coach, and develop clinical teams.
Strong organizational, problem-solving, and decision-making skills.

Ability to manage multiple priorities while maintaining attention to clinical quality and patient safety.
Comfortable using electronic medical record systems and other healthcare technology.
What We’re Looking For
Beyond clinical experience, we are looking for someone who is proactive, approachable, organized, and willing to lead from the front.
