Join High Desert Medical Group as a Clinical Pharmacist – Disease Management in California.
Key Responsibilities
- Comply with state and federal drug laws as regulated by the California State Board of Pharmacy, the Drug Enforcement Administration, and the Food and Drug Administration.
- Medication reconciliation.
- Perform Disease Management Programs duties, including but not limited to contacting the patient(s) medication dosage titrations, and diabetic panel parameters assessment.
- Consult patients in hour and IPA physicians on matters concerning diabetes.
Requirements
- Doctorate in Pharmacy required.
- Hospital Pharmacy Residency or comparable pharmacy experience preferred.
- Ability to follow and adhere to FDA Health Regulations, and other legal compliances.
- Excellent written and verbal communication skills.

- Salary: $135,200 - $180,232/year
