Key Responsibilities

Conduct comprehensive in-home health evaluations to bridge gaps in care and build trusted relationships with patients. Perform medication reviews, physical assessments, and diagnostic screenings while documenting findings through an intuitive clinical workflow.

Requirements

Must hold an active, unrestricted MD or DO license in the coverage area. Ability to travel locally and within licensed states is required for home visits.
