Key Responsibilities

• Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.

• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.

• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.

Requirements

• Social Worker Requirements & Core Responsibilities: Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.
