Our interdisciplinary team serves patients wherever they call home: private residences, assisted living communities, and skilled nursing facilities across the Denver metro area.

The Role

  • Connect families with community resources, including Medicaid, VA benefits, caregiver support, and placement options
  • Take part in Interdisciplinary Group (IDG) meetings and contribute to individualized plans of care
  • Identify caregiver stress, safety concerns, and risk factors, and escalate them appropriately
  • Coordinate with our bereavement team to support families after a death
  • Document visits accurately and on time in the EMR, in line with Medicare and CHAP standards

What We’RE Looking For

  • Master’s degree in Social Work (MSW)
  • Current Colorado license (LSW or LCSW); LCSW preferred
  • Hospice, palliative care, home health, or medical social work experience preferred
  • Strong assessment, crisis intervention, and communication skills
  • Comfort working independently in the field and managing a caseload
  • Valid Colorado driver’s license, reliable transportation, and current auto insurance
  • Proficiency with electronic medical records

Schedule & Compensation

  • Pay: $30–$35 per hour / per visit, depending on experience and licensure
  • Mileage stipend: $180/ Month

Why Ovation?

  • Meaningful work that makes a real difference for patients and families
  • A supportive, mission-driven interdisciplinary team
  • Flexible scheduling built around your life
  • [Add any differentiators: CEU support, orientation and mentorship, team culture]
