Key Responsibilities
The Medical Social Worker provides clinical support and psychosocial assessments to help patients manage the emotional and social challenges of chronic kidney disease. They also collaborate with a multi-disciplinary team to develop care plans and educate patients on treatment options to improve their quality of life.
Requirements
Candidates must hold a Master's Degree in Social Work from an accredited CSWE institution and possess the required state licensure (LCSW/ACSW/LMSW). At least one year of experience in a behavioral or healthcare-related setting is preferred.
