The Behavioral Health Consultant identifies and addresses psychosocial factors affecting physical health through brief interventions and short-term counseling. They collaborate within a multidisciplinary primary care team to connect patients, particularly those experiencing homelessness, to essential community resources.
Candidates must hold a Master's or Doctorate in Counseling, Social Work, or Psychology and possess a valid Arizona license (LPC, LCSW, or Psychologist). A minimum of two years of healthcare experience and a current Arizona Driver License and BLS certification are required.
