Key Responsibilities

Implement rehabilitation plans of care for home health patients and conduct routine re-evaluations of therapy needs. Coordinate with physicians and clinical staff to ensure continuity of care and prevent avoidable hospitalizations.

Requirements

Must be a graduate of a certified occupational therapy curriculum with a current, unrestricted COTA certification. Requires at least one year of clinical experience and a valid driver's license for travel within the coverage area.
