Key Responsibilities
The Palliative Care Coordinator provides psychosocial support to patients and facilitates goal setting and care planning within the palliative care team. They also act as a liaison between the palliative care team, hospital departments, and community services to ensure coordinated patient care.
Requirements
Candidates must hold a bachelor's degree in nursing, social work, or a related field and possess at least 3 years of experience with chronic or life-threatening illnesses. A valid New Jersey professional license as an RN, LSW, or LCSW is required.
