Key Responsibilities

The RN Case Manager collaborates with interprofessional teams to facilitate effective transitional care coordination and ensure continuity of care. They are responsible for conducting holistic assessments, developing individualized care plans, and ensuring compliance with regulatory standards.

Requirements

Candidates must be graduates of an accredited school of nursing with current state licensure and at least three years of recent RN experience. A Basic Life Support (BLS) certification is required, while a BSN and relevant certifications like CCM or ACM are preferred.
