Performs routine physical/psychosocial assessment to collect basic data: follows the written plan of care, standards, practice guidelines, utilizes pathways, which includes notifying physicians as appropriate
Formulates a plan of care for individual needs, including patient population needs. Participates in the planning of routine transitional health care needs (i. e. treatment options, patient placement options)
Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content. Integrates education during the delivery of patient care
Ensures efficiency in the delivery of care to determine immediate priorities and to offer solutions for problems
- Makes independent clinical decisions based on knowledge drawn from education and experience
- Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict

- Graduate of an accredited school of Nursing and FNP
- Minimum one year of experience as a Family Nurse Practitioner in an outpatient clinic setting
RN NP BLS and DEA Certifications
Active licensure as a registered nurse in California or possession of a temporary California RN license (TL) a California (California Code of Regulations Section 1482) and Licensed as a Nurse Practitioner in good standing from the California State Board of Nursing.
National Nurse Practitioner Certification in good standing from either the: AANP (American Academy of Nurse Practitioners) or ANCC (American Nurse Credentialing Center).
Key Responsibilities
The Family Nurse Practitioner will perform physical and psychosocial assessments, formulate individualized patient care plans, and collaborate with an interdisciplinary team to ensure optimal outcomes. They will also provide patient education, make independent clinical decisions, and advocate for patient rights within the hospital setting.
Requirements
Candidates must be graduates of an accredited nursing school with a Family Nurse Practitioner certification and at least one year of outpatient clinic experience. Required credentials include active California RN and NP licensure, BLS certification, DEA certification, and national certification from AANP or ANCC.
