Responsibilities

  • Proactive, collaborative team member in a hospice care environment
  • Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
  • Commitment to caring for patients and partners
  • Complete all documentation and records in a timely and accurate manner
  • Follow established policies and procedures as related to the scope of practice
  • Maintain continuity of patient care in a hospice setting

Licensure, Certification, and Educational Requirements

Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice

Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care

Requirements

Must hold current, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in the state of practice. Board certification by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care is required.
