The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities
GENERAL PURPOSE:
Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.
FT position available, primarily M-F days, evening and weekends per unit needs.
Joining the Hospital at Home team is an exciting opportunity to be part of an innovative care model that provides high-quality, patient-centered acute care in the home setting. The Nurse Navigator plays a crucial role in guiding patients from admission through the continuum of care while participating in the Hospital at Home program.
Responsibilities include utilizing screening tools to identify and select appropriate patients for the program. The RN Navigator works directly with patients and families to provide education about Hospital at Home, support informed decision-making, and successfully enroll eligible participants.
The RN Navigator identifies each patient’s individual needs and potential barriers to successful participation in the program and collaborates with the interdisciplinary team to address and mitigate those barriers.
The RN Navigator coordinates the admission process and helps ensure a safe and seamless transition from the hospital to the patient’s home setting.
This role works primarily within the hospital setting and collaborates closely with the Hospital at Home nursing and provider teams, inpatient nursing units and provider teams, and external partnership teams to coordinate patient care and support successful transitions to the home.
The successful candidate will possess strong communication and organizational skills and demonstrate the ability to work independently, anticipate needs, and proactively solve problems.
A BSN and a minimum of three years of acute care, ICU, and/or Emergency Department experience are required. RN navigation or care coordination experience is preferred. This position requires a valid NYS driver’s license and reliable transportation
