Key Responsibilities
- will be actively engaged in all complexities of patient medication management, monitoring, and education, as needed.

- Illinois Department of Financial and Professional Regulation (IDFPR) And Board Certified Ambulatory Care Pharmacist (BCACP)
- Board of Pharmacy Specialties (BPS) Or Board Certified Cardiology Pharmacist (BCCP)
