Key Responsibilities

• Maintains accurate, complete and timely documentation reflecting the examination, evaluation, diagnosis, prognosis, plan of care, education and interventions.

• Evaluates patient progress toward functional goals, and will modify the the plan of care accordingly.

• Provides patient, family, and clinician education regarding the patient s functional status and plan of care.

• Performs other duties as assigned

Requirements

• Master's Degree. Current and valid BLS or CPR certification from the American Heart Association - Outpatient facility only. 1- 2 years Hospital (Inpatient) or Orthopedic (Outpatient) Rehabilitation setting. Experience in Neuro - Outpatient facility only.
