Mental Health Kokua (MHK) is hiring a CIS Homeless Outreach Case Manager to join our West Hawaiʻi team in Kona. This field-based role focuses on engaging adults experiencing homelessness and serious mental illness and connecting them with housing, healthcare, and essential community services.

Key Responsibilities

• Client Engagement: Conduct field

• based outreach to build trust with adults experiencing homelessness and serious mental illness

• Caseload Management: Maintain a caseload of up to 15 clients and meet with each client regularly to support service goals

• Assessment & Referrals: Assess needs and coordinate referrals for psychiatric, medical, and housing services

Requirements

• Data Management: Enter and maintain client data in HMIS and other required systems

• Insurance Verification: Confirm insurance coverage and payer authorizations as required

• Team Participation: Attend supervision, trainings, and team meetings to ensure quality, person

Benefits & Compensation

• Hourly rate: $27/hr.

• PSLF eligible (Public Service Loan Forgiveness).
