Provide in-home physical therapy services to help patients restore function, relieve pain, and prevent disability. Collaborate with physicians and interdisciplinary teams to develop and implement individualized treatment plans.
Requires a valid PT license, BLS certification, and 1-2 years of general PT experience. Must have at least one year of recent Home Health experience and proficiency in Home Care Home Base (HCHB) charting.
