• Short-Term Rehabilitation and Skilled Geriatric Care facility sponsored by the
• Carmelite Sisters for the Aged and Infirm, we've served the community since

home for our residents.

We are seeking a Social Worker - for a TEMPORARY assignment.

* $40 per hour.

* Full-Time; Monday - Friday schedule.

Social Worker Qualifications

* Must have a Bachelor's Degree in Social Work or related field.

* One year of Social Work in a Nursing Home required.

* Must have MDS assessment experience.

* Experience using PCC is a plus.

Social Worker Introduction

Under the supervision of the Director of Social Work, assist the residents to

attain or maintain the highest practicable physical, mental and psychosocial

well-being, and in navigating the health care system.

Social Worker Job Summary

* Interview residents and their families.

* Provide assessments and counseling to residents and families.

* Develop care plans based on the individual need of residents.

* Provide crisis intervention as needed.

* Provide grief and supportive counseling.

* Coordination of discharge planning and case management.
