Key Responsibilities
The Case Manager coordinates skilled care for residents according to physician orders and manages communication between the interdisciplinary team, residents, and families. They are responsible for conducting pre-admission assessments, overseeing MDS documentation, and participating in quality assurance processes.
Requirements
Candidates must be a Registered Nurse with a current license from the State Board of Nursing and graduation from an accredited nursing school. CPR certification is recommended for this role.
