Key Responsibilities
The physician will provide value-based primary care to a senior population within an interdisciplinary team environment. Responsibilities include managing patient panels, performing health maintenance, and coordinating care through referrals and disease prevention strategies.
Requirements
Candidates must hold a Doctor of Medicine or Osteopathic Medicine degree and be board certified or eligible in Geriatric, Family, or Internal Medicine. A minimum of two years of experience, including at least one year managing a Medicare Advantage panel, is required along with an unrestricted medical and DEA license.
