Assess patients' medical and mental health needs to develop and implement proactive care plans. Collaborate with an integrated care team, including RN case managers and facility partners, to manage patient conditions and prevent unnecessary hospital visits.
Requires an FNP, AGNP, AHACNP, or PA license and a degree from an accredited APRN or PA program. Candidates must have at least 3 years of clinical experience in geriatric populations and proficiency with electronic health records.
