The Nurse Practitioner will provide hospice and palliative care to seriously ill patients to alleviate symptoms and align treatments with patient values. They will collaborate with a clinical team and provide ongoing education and support to patients and their families in their homes.
Candidates must be a graduate of an accredited nursing program and possess at least 2 years of experience in chronic disease management or long-term care. A valid driver's license and reliable transportation are required, along with the physical ability to lift 80-100 lbs.
