Qualifications
Qualifications Active RN License required Hospice nursing experience preferred Experience with Homecare Homebase (HCHB) preferred Strong assessment, communication, and care coordination skills Schedule & Work-Life Balance Monday–Friday, 8:00 AM – 4:30 PM On-call opportunities available with additional compensation Some locations offer dedicated on-call teams, resulting in backup coverage only Key Responsibilities: Provide skilled nursing care, assessments, and sym
- ptom management in accordance with hospice standards, physician orders, and individualized plans of care. Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB). Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care. Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes. Educate patients, families, and caregivers on sym
- ptom management, disease progression, medications, and end-of-life care while providing compassionate support.
Key Responsibilities
Provide hospice nursing assessments, skilled care, symptom management, scheduled visits, admissions, and clinical interventions in accordance with physician orders and individualized care plans, documenting care in Homecare Homebase. Coordinate with physicians, caregivers, and the interdisciplinary team; respond to patient and family needs, including on-call situations; and educate and support patients, families, and caregivers through end-of-life care.
Requirements
An active RN license is required, and hospice nursing experience and familiarity with Homecare Homebase are preferred. Candidates should have strong assessment, communication, and care coordination skills.
