If you hold an undergraduate B. A, Degree in Social Work, you are eligible to apply. Please know the hirable rate of pay will be based on degree type. Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred Must understand hospice philosophy, and issues of death/dying. Key Responsibilities
The Social Worker will assess the psychosocial status of hospice patients and their families to provide support, crisis intervention, and care planning. They act as a liaison between patients and community agencies while maintaining clinical records and participating in interdisciplinary team meetings.
Requirements
Candidates must hold a master's degree in social work, though a bachelor's degree is also accepted. A minimum of two years of documented supervised experience in health care or hospice is required.
