Job Description

Work Schedule

This is a full time, 100% FTE position, day/evening. Monday – Friday between the
hours of 12:00pm – 8:00m, no weekends required. Hours may vary based on the
operational needs of the department.

This position will provide primary onsite coverage for Union Corners Urgent Care
daily, with remote support provided to West Towne Urgent Care.

We are seeking a Social Worker – Urgent Care to:

* Evaluate complex patient and family situations which have legal, ethical, and
psychosocial components in a fast-paced, episodic care environment.
* Collaboratively develop patient goals, provide brief interventions, address
immediate barriers, and support safe and appropriate care transitions or
follow-up.
* Assess and respond to social drivers of health impacting urgent care
utilization, including housing instability, food insecurity, transportation
barriers, financial strain, limited access to primary care, and safety
concerns.
* Provide resource connection and care coordination to address immediate needs,
reduce barriers to discharge, and support linkage to community services and
ongoing care.
* May engage patient/family in the treatment and decision-making process with
respect to patient rights, principles of confidentiality, respect for patient
privacy, and right to self-determination.

Qualifications
* Master's Degree in Social Work Required

Work Experience
* 2 years prior hospital based social work experience Preferred
* 2 years prior hospital or community-based social work experience if
supporting the HIV Clinic Preferred
* 2 years prior hospital or clinic based pediatric experience if supporting
American Family Children’s Hospital (AFCH) Preferred

Licenses & Certifications
* Certified Advanced Practice Social Worker (CAPSW) in the state of Wisconsin
within 90 Days Required or
* Certified Independent Social Worker (CISW) in the state of Wisconsin within
90 Days Required or
* Licensed Clinical Social Worker (LCSW) in the state of Wisconsin within 90
Days Required
