About the Role

Join UW Health as a Social Worker, Urgent Care in United States. Work Schedule: This is a full time, 100% FTE position, day/evening. Monday – Friday between the hours of 12:00pm – 8:00m, no weekends required.

Key Responsibilities

provide primary onsite coverage for Union Corners Urgent Care daily, with remote support provided to West To.

develop patient goals, provide brief interventions, address immediate barriers, and support safe and appropr.

Provide resource connection and care coordination to address immediate needs, reduce barriers to discharge,.

Requirements

Master's Degree in Social Work Required Work Experience

2 years prior hospital based social work experience Preferred

2 years prior hospital or community

based social work experience if supporting the HIV Clinic Preferred
