About the Role
Join UW Health as a Social Worker, Urgent Care in United States. Work Schedule: This is a full time, 100% FTE position, day/evening. Monday – Friday between the hours of 12:00pm – 8:00m, no weekends required.
Key Responsibilities
- provide primary onsite coverage for Union Corners Urgent Care daily, with remote support provided to West To.
- develop patient goals, provide brief interventions, address immediate barriers, and support safe and appropr.
- Provide resource connection and care coordination to address immediate needs, reduce barriers to discharge,.

Requirements
- Master's Degree in Social Work Required Work Experience
- 2 years prior hospital based social work experience Preferred
- 2 years prior hospital or community
- based social work experience if supporting the HIV Clinic Preferred
