We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Palmyra, PA area. This position is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid after 30-days of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!

What You’ll Do:
Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
Work independently, but never aloneTravel to patient homes within assigned territory

PT Qualifications
Active Physical Therapist, PT license in the state you work
One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
