Job Description
Empath Home Health, a member of Empath Health, is seeking a caring and motivated PRN Home Health Occupational Therapist (OT). The Home Health Occupational Therapist provides exceptional care to our clients in Lee County (Fort Myers) area.

What You'll Do
- Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
- Performs initial and on going assessments to determine level of functioning and patient’s ability to perform activities of daily living, including OASIS assessments at appropriate time points.
- Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care.

What You'll Need
- Bachelor Degree in Occupational Therapy.
- Florida licensure in occupational therapy.
- Minimum of one (1) year experience as a licensed Occupational Therapist.
- Home health care experience preferred.

Key Responsibilities
The Occupational Therapist will perform patient assessments, including OASIS, and develop individualized plans of care to help patients achieve self-care goals. They will provide treatment in compliance with agency policies and federal/state regulations while collaborating with physicians and the care team.
Requirements
Candidates must hold a Bachelor's degree in Occupational Therapy and a valid Florida license. A minimum of one year of experience as a licensed Occupational Therapist is required, with home health experience preferred.
