About the Role

Join Empath Health as a Occupational Therapist – Home Health PRN, Fort Myers in United States. Empath Home Health, a member of Empath Health, is seeking a caring and motivated PRN Home Health Occupational Therapist (OT). The Home Health Occupational Therapist provides exceptional care to our clients in Lee County (Fort Myers) area.

Key Responsibilities

Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.

Performs initial and on going assessments to determine level of functioning and patient’s ability to perform activities of daily living, including OASIS assessments at appropriate time points.

Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self
