Social Worker

Key Responsibilities

This role involves coordinating admissions, assessing patient needs, sharing funding resource information, and working with patients and families through crises like illness, disability, and death. The social worker will also advocate for patients, coordinate discharge planning, and participate in developing patient care plans.

Requirements

Candidates must possess knowledge of the case management process, the needs of geriatric patients, and healthcare settings, including working knowledge of funding resources like Medicare and Medicaid. A Bachelor's degree in Social Work or another related human services field from an accredited institution is required.
