Key Responsibilities

Conduct in-home health evaluations to bridge gaps in care and provide comprehensive health assessments for patients. Engage with individuals to review medical history, perform physical evaluations, and facilitate connections to appropriate care services.

Requirements

Must be a licensed Physician (MD or DO) with an active, unrestricted license in the coverage area. Ability to obtain and maintain multi-state licenses and travel up to 100% of the time is required.
