Provide one-on-one physical therapy to older adults in private residences and senior living communities to improve mobility and independence. Perform evaluations, develop individualized care plans, and utilize telehealth services to extend patient access.
Must hold a valid Physical Therapy license (or eligibility) and a degree from an accredited Physical Therapy program. Requires current CPR certification and the physical ability to assist with patient transfers and lift up to 50 lbs.
