- Home Health Physical Therapist (PT) | Oʻahu, HI
- Pay: $80 per visit
- Schedule: Flexible / Per Diem – Choose your patients
- Location: Oʻahu, Hawaii (reliable transportation required)

Always Best Care is a Medicare-certified home health agency committed to helping patients remain safe, independent, and active in their own homes. Our therapy team plays an essential role in improving mobility, strength, and overall quality of life for the seniors we serve.
We are currently seeking licensed Physical Therapists to provide home health services across Oʻahu.

Key Responsibilities
- Provide skilled physical therapy visits in patients’ homes
- Conduct PT evaluations and follow-up assessments
- Develop individualized treatment plans focused on mobility, strength, balance, and fall prevention
- Educate patients and family caregivers on safe mobility techniques
- Maintain accurate, Medicare-compliant documentation
- Collaborate with the care team to support patient progress
Qualifications
- Current Hawaii Physical Therapist (PT) license
- Minimum 1 year clinical experience (home health experience preferred)
- Strong documentation and communication skills
- Ability to work independently in a home setting
- Valid driver’s license and reliable transportation
