Job Description

Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter. Performs mini-mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a time frame compliant with company, state, and federal guidelines. Is responsible for contacting any patient who is absent for 3 or more days to determine reason for the absence and when they plan to return to the program.

Key Responsibilities

The Crisis Social Worker assesses patients, develops treatment plans, and facilitates individual and group therapy sessions. They are also responsible for documenting all interactions and monitoring patient attendance to ensure continuity of care.

Requirements

Candidates must hold a Master's degree in Social Work or a related field and possess at least two years of relevant experience. Additionally, current professional licensure and CPR certification are required.
