The Transplant Nurse Case Manager will conduct outbound calls to assess member health status, identify treatment gaps, and provide patient education. They will also facilitate referrals and coordinate care resources to support members with solid organ transplants.
Candidates must hold an unrestricted RN license and have at least 3 years of experience in an acute care or hospital setting. Additionally, 1 year of experience working with solid organ transplant or VAD patients is required.
