Key Responsibilities

The Physical Therapist evaluates, plans, and implements individualized treatment programs for patients in their homes. They also coordinate care with the clinical team and educate patients and families on safety and mobility.

Requirements

Candidates must hold an active Physical Therapist license in Ohio and possess a valid driver's license with reliable transportation. A minimum of one year of experience, a current TB test, and CPR certification are preferred.
