The RN Case Manager facilitates patient care throughout hospitalization by reviewing resource utilization and ensuring appropriate levels of care. They collaborate with the multidisciplinary team to develop and implement effective discharge plans based on individual patient needs.
Candidates must be a graduate of an accredited nursing school and hold a current registered nurse license in the state of practice. Basic Life Support (BLS) certification from the American Heart Association is also required.
