The Social Worker assists with the acclimation of admission for each resident as well as discharge planning upon leaving the facility.

Social Worker Qualifications: High School Diploma or equivalent Within the last five years, must have one year of supervised social work experience (preferably in a healthcare setting) Must have valid Virginia driver's license
Key Responsibilities
The Social Worker is responsible for assisting with the acclimation of new residents upon admission and managing discharge planning when residents leave the facility. They also provide direct support to residents and their families while acting as a resident advocate.
Requirements
Candidates must possess at least a High School Diploma or equivalent and have one year of supervised social work experience within the last five years, preferably in a healthcare setting. A valid Virginia driver's license is also required.
