Join Western CT Home Care as a Registered Dietitian Per Visit in Danbury, Connecticut. Plans, organizes, and conducts nutrition consults to home health patients based on patient need and physician orders. Functions as a member of the home care team with expertise in nutritional counseling.

Key Responsibilities

• Communicates effectively with all team members, patients/patient representatives/caregivers through conferences, individual consultation, and committee participation. This includes telephone, written, and electronic documentation.

• Maintains confidentiality and safety of patient clinical records and information.

• Attends case conferences and staff meetings as appropriate.

• Complies with the organizations' policies and procedures.

Benefits & Compensation

• Pay rate: hour9.65 - $55.55/hour.

Work Schedule

• Per Diem position.
